Community Allied Mobile Palliative Partnership

Through collaborative partnership with Alberta Health Service (AHS) Palliative and End of Life Program and the AHS Palliative Homecare Team, the Community Allied Mobile Palliative Partnership (CAMPP) provides palliative and end-of-life care for individuals who are unstably housed or homeless.

The program offers intensive case management, physician facilitated pain management, healthcare navigation, mental health and addiction support, and housing assistance to ensure individuals with life-limiting illnesses receive compassionate, dignified care.

Who can access this program? 

Referrals must be made internally through CUPS or by external partner organizations or health providers. Services are available to individuals diagnosed with a life-limiting, life-threatening, or palliative illness, who are experiencing homelessness or unstable housing, or living on a low income.
