Connect 2 Care
Connect 2 Care is a mobile team supporting individuals experiencing homelessness by helping them transition from frequent hospital and acute care use to community-based healthcare. The program provides short term, intensive case management, advocacy and support with navigating systems of care to promote long-term stability and well-being.

Who can access this program?
Referrals must come from a healthcare or community care provider. To qualify, individuals must have had at least six (6) emergency room visits or two (2) hospital stays in the past 12 months, be experiencing homelessness or unstable housing, and meet low-income criteria.

How does Connect 2 Care team help individuals with high acute care use?
This program connects individuals to healthcare, housing, mental health support, and financial resources to transition from hospital care to stable community services. High acute care use means six ER visits or two hospital admissions in the past 12 months.
Does Connect 2 Care provide long-term housing?
The program helps individuals secure stable housing through referrals and advocacy but Connect 2 Care is not a direct housing program.
Is mental health support included?
Yes, Connect 2 Care provides mental health, addiction support, and system navigation services to support recovery and stability.
