KEY CASE MANAGEMENT

 

CUPS Key Case management focuses on maintaining long-term housing stability and improved well-being (physical and mental health). The goal is independence and community re-integration.

Our Case Managers support up to 120 adults who have experienced chronic or episodic homelessness. It includes:

  • access to permanent housing with no barriers or housing readiness (subsidy provided)

  • developing individualized service plans

  • enhancing life skills and addressing health and mental health needs

  • building social and community connections by engaging in activities that are meaningful to the resident

All program referrals are from CAA Placement committee meetings.
